Medical Records 

The Medical Records Department is responsible for maintaining medical records in a standardized and professional manner to protect patient confidentiality while allowing adequate access to providers in order to promote quality patient care.

Records are released in accordance with the Health Insurance Portability and Accountability Act 1996 (HIPAA), New York State and Federal Laws.

How to Request a Copy of Your Medical Records
Requests for medical records must be done in person. Current picture identification, with signature, must be presented.  A processing fee of 75 cents per page is required.  Medical Record copies are processed within 5-10 business days after requests have been submitted. 

Under the Age of 18:
Parents and legal guardians have the right to request dependent children's records before the age of 18 years by completing a signed request form with proper identification. 

Certified Birth Certificates are NOT available from the MTVNHC Medical Records Department.  These records must be ordered from the Department of Vital Statistics in the state which the birth was registered. 

Patients have the right to:

Access or inspect their health record
Obtain a copy from their healthcare provider 
Access and copying for as long as information is retained
Inspect and get a copy of the Notice of Privacy Practices (NOPP)
Request an amendment
Request restrictions and limitations on disclosures
Receive and obtain copies of the Protected Health Information (PHI)
Complain about your Privacy Practices

Medical record request for continuum of care are sent to other providers without a fee.

The Medical Records Department is open during regular business hours at each Network site location.
