Patient Registration
Patients must register at the Center before receiving care. The registration procedure is completed in the Registration Office. Registration personnel will request information regarding insurance (Medicaid, Medicare, or other insurance); so it is important to bring insurance information with you to registration.
For those patients without insurance coverage, registration personnel will also request a recent pay stub. patient registration hours are as follows:
