Patient Rights & Responsibilities
To achieve and maintain health standards for all patients, the Mount Vernon Neighborhood Health Center, in partnership with each patient, has adopted the following principles for patient treatment and patient responsibility.
Rights
- The health Center will not discriminate in providing medical treatment because of age, sex, religion, color, national origin or source of payment.
- Each patient will be given all necessary facts before agreeing to any examination or treatment, and any patient may refuse such examination or treatment to the extent permitted by law.
- Each patient should expect the Health Center Staff to make reasonable and courteous responses to requests for services.
- The confidentiality of the patient's medical care program will be protected by the Health Center.
- The names and specialities of the physicians responsible for a patient's care will be made available to the patient.
- Each patient is encouraged to ask information concerning available services at the Health Center.
Responsibilities
For optimal care, it is necessary that each patient:
- Supply the Health Center with complete information about past illnesses, hospitalizations, medications and othe matters relating to health.
- Cooperate with the Health Center Staff and advise immediately if instructions are not understood or cannot be carried out.
- Keep appointments and promptly notify the Health Center if an appointment must be cancelled.
- Inform doctors and nurses of any unexpected change in condition.
