Social Work Department

Welcome to the Mount Vernon Neighborhood Health Center Network Department of Social Service

The Social Work Department team is informed and knowledgeable of the latest news and developments in social work practice, education and research to better serve the diverse needs of our clients.  The department is comprised of experienced and qualified social workers and provides services to all ages, from children to adults. Working as part of the client's healthcare team, our professional social work staff helps patients and families understand and cope with medical, physchological and social issues. 

Counseling can assist patients and families address concerns about illness, loss, personal crisis and adjusting to lifestyle changes. 

Services provided are as follows:

  • Psychotherapy – short and long term therapy
  • Help patients access home health aid services, visiting nurse services, personal care services
  • Make referrals to outside agencies i.e. Social Security Department, Section 8, Alcohol/Drug Rehabilitation, Food Pantries
  • Help disabled patients access transportation services
  • Case Management
  • Help pregnant mothers apply for the health insurance through Prenatal Care Assistance Program (PCAP)
  • Help patients receive authorization for enteral formulas i.e. Ensure, Pediasure, Glucerna
  • Help patients who need financial assistance in paying for their medications
  • Assist with senior housing placement
  • Adoption counseling and referral

Monday - 10:00am - 6:00pm
Tuesday - 10:00am - 6:00pm
Wednesday - 10:00am - 6:00pm
Thursday - 10:00am - 6:00pm
Friday - 10:00am - 6:00pm
